All-on-4 Case: How Long Until Fixed Teeth? When Waiting Is Part of Treatment
Summary: The plan for this All-on-4 patient was fixed teeth on the day of surgery. Four Neobiotech implants were placed. Three reached good stability, but the last one did not, because the bone in that area was too soft. Dr. Nalat chose to let the implants heal for about four months, with a removable denture in the meantime, rather than move the implant to a less suitable position. Four months later the patient received the fixed teeth as planned.
“How long until I get my fixed teeth after All-on-4?”
This is one of the questions patients ask most. Dr. Nalat’s honest answer is: “I will plan for fixed teeth as soon as it is safe, but I can’t promise they will go in on the day of surgery, because several things have to come together.”
Not everyone leaves with fixed teeth on surgery day. This case shows why.
What happened during surgery
- Implant 1: good stability ✓
- Implant 2: good stability ✓
- Implant 3: good stability ✓
- Implant 4: not stable enough, because the bone quality in that area was poor
To attach fixed teeth on the same day (immediate loading), every implant needs enough initial stability. With one implant below that level, there were two options:
- Wait about 4 months for the implants to bond with the bone, using a removable denture in the meantime
- Move the implant to a different position
Assessment during surgery showed that the other positions were unlikely to give better stability, and might not be ideal for long-term function. So the decision was to wait.
Why waiting was the right call here
Loading an implant that is not stable enough can put it at risk of failing. Waiting a few months adds time, but it protects the long-term result. Because this possibility had been discussed with the patient before surgery, the patient understood why the plan changed.
The result
After about four months of healing, the fixed All-on-4 bridge was fitted. The patient now chews comfortably, as intended from the start, even though it took longer than the first plan.
A beautiful smile in a photo can make treatment look quick and easy. Behind it are details, limits and decisions. Sometimes waiting is part of the treatment.
What this means if you are travelling for All-on-4
This patient lives in Thailand. If you are coming from abroad, plan your trip with this possibility in mind. When the bone allows, fixed temporary teeth can be attached on the day of surgery. If the bone does not allow it, you would leave with a removable temporary denture and return for the fixed teeth after healing, usually around 3–6 months later. Sending a CBCT scan before you travel helps the team tell you how likely same-day teeth are for you.
Questions about All-on-4 timing
Do all All-on-4 patients get teeth on the day of surgery?
No. Same-day fixed teeth depend on how firmly each implant is held in the bone during surgery. Bone density, bone volume and the position of each implant all play a part. The final decision is made during surgery.
What happens if I can’t have fixed teeth straight away?
You wear a removable temporary denture while the implants heal, usually for about 3–6 months. After that, the fixed bridge is attached.
