Full Mouth Rehabilitation Case: Implants with Preserved Natural Teeth (Patient from Australia)
Summary: A patient from Australia came to Smile and Shine with several decayed and broken teeth. Instead of removing all the teeth for an All-on-4, Dr. Nalat’s team kept the natural teeth that were still healthy and replaced only the teeth that could not be saved. Treatment was completed in two trips to Bangkok, four months apart: Neobiotech implants with bone grafting on the first trip, then ceramic crowns on the implants and the natural teeth on the second. Each trip took about three weeks.
Not every full mouth case needs All-on-4
Many people assume that when several teeth are decayed or broken, the only option is to extract everything and have a full arch on four implants. All-on-4 is a good fit when:
- most or all of the teeth are already missing
- the remaining teeth cannot be kept in a healthy condition
- so many teeth need extraction that a full-arch bridge on 4–6 implants is the practical solution
When some natural teeth are still strong, a combined plan is often worth considering.
Why keep healthy natural teeth?
- The bone and gum shape around natural teeth is preserved
- It helps limit bone loss in the jaw
- Chewing feels natural
- Fewer implants and less surgery may be needed, which can lower the overall cost
“Preserve what is healthy. Replace only what is necessary.”
The patient’s situation
The patient had multiple decayed and fractured teeth, but some natural teeth were in good condition and could be kept. The team assessed every tooth with a clinical examination and a 3D CBCT scan before planning.
Treatment plan: two trips to Bangkok
Trip 1 (about 3 weeks): Implant surgery, bone grafting and extractions
- Digital treatment planning with a 3D CBCT scan
- Extraction of the teeth that could not be saved
- Dental implants (Neobiotech, Korea) placed in the planned positions
- Bone grafting to support the future teeth
Healing at home: about 4 months
The patient returned home to Australia while the implants bonded with the jawbone.
Trip 2 (about 3 weeks): Ceramic crowns on implants and natural teeth
Once the implants were stable, ceramic crowns were made for both the implants and the preserved natural teeth, so the whole smile matches in colour and shape. The patient then used the new teeth for a while in Bangkok and came back for a recheck and any fine adjustments before flying home.
The result
- Strong, natural-looking teeth
- Better chewing function
- No unnecessary extractions or surgery
- Treatment cost kept to the agreed plan
Planning dental treatment in Bangkok from abroad?
For international patients, the plan is usually organised around trips: assessment, CBCT and surgery on the first visit, then the final crowns after healing. The number of trips and days in Bangkok depend on your bone, the number of implants and whether grafting is needed. You can send your X-rays or photos before you travel so the team can give a preliminary plan.
Questions about this treatment
Should I extract all my teeth and get All-on-4?
Not necessarily. If some of your teeth are healthy, keeping them and placing implants only where teeth are missing may be an option. A full examination and CBCT scan show which teeth can be kept.
How many trips to Bangkok are needed?
In this case, two trips four months apart: about three weeks for surgery and grafting, then about three weeks for the final crowns, time to get used to the new teeth, and a recheck before flying home. Your own plan depends on your bone and treatment needs.
How much does full mouth rehabilitation cost?
The cost depends on how many implants, crowns and grafts are needed. See the dental implant cost page for implant prices, or send your X-rays for an estimate.
